Blueprint Baseball Terms and Conditions

DUTY TO WARN
I fully understand that there are risks and dangers inherent in and associated with participation in sport camp/clinic-related events and activities and may result in bodily injury, partial and/or total disability, paralysis and death.

Examples of sport camp-related events and activities are, but not excluded to:

Participating with standard baseballs or hardballs
Particpating in impact skills such as sliding or diving
Participating in hitting activities, which may result being hit by a thrown/pitched ball
Participating in fielding and throwing activities which may result in being struck by a moving ball
Participating in water-related activities (e.g. portable water slides)
Participating in an egg toss competition
WAIVER OF LIABILITY/HOLD HARMLESS
In enrolling at Blueprint Baseball, the said participant understands that he/she attending the programs and using the facilities of Blueprint Baseball and host/partner organizations does so at his/her own risk. Blueprint Baseball and its host/partner organizations and their owners, employees or agents, shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by participant with his/her family in or about any programs on the premises. Participants and parents assume full responsibility for all injuries and damages which occur in or about any programs on the premises, He/She does hereby fully and forever release discharged hold harmless Blueprint Baseball, all associated facilities and its owner, employees, and agents from any and all claims, demands, damages or rights of action, present or future resulting from any person’s participation in any programs or use of the facility. In addition, he/she agree(s) to follow the rules of conduct and play set by Blueprint Baseball. Failure to do so may result in suspension from participation.

LICENSE TO TREAT INJURED PERSONS
I, the undersigned parent or guardian/participant, do hereby grant authority to the staff at Blueprint Baseball and/or participating host/partner organizations, to render judgment concerning medical assistance or hospital care in the event of an accident or illness during my absence. I hereby give my consent to Blueprint Baseball and any participating host/partner, in the event of possible injury to my child, to provide, through a medical staff of its choice and within staff members' training and scope, customary medical/athletic training attention, transportation and emergency medical services as warranted in the course of my child's participation in events sanctioned by Blueprint Baseball and participating host/partner members.

REGISTRATIONS & PAYMENTS
I understand that payment in full is due upon the time of completed registration. I understand that any and all discounts applicable to my registration must be claimed at the time of the registration or I may forfeit any and all such applicable discounts. I understand that in the event of a discount awarded for the registration of multiple clinics, children, etc. my discount applies to the smallest dollar amount.

I do hereby authorize Blueprint Baseball and its host/partners to utilize any and all videography and photography, pictures or other likeness of me or anyone assigned guardianship to me, as they deem appropriate for use in its promotional materials or team films.
